DOES YOUR CLINICAL RECORD SUPPORT YOUR CHARGES?

It is the responsibility of each provider to document care in the progress note to support all services that were provided to the patient. If the service was provided but was not documented in clinical record then technically the service was not provided. During an audit the insurance carrier has the right to request a refund for all services not supported in the clinical record.

In healthcare communicating with patients and ensuring that patient’s return for follow up visits has become a full time job for most clinics. The liability of missed appointments in many cases falls on the responsibility of each clinic. A simple missed appointment may go unscheduled for a patient for an indefinite amount of time. For some patients with chronic health conditions the failure to comply with follow up appointments may lead to increased health risks and morbidity.

Automatic Conversion from ICD9 to ICD10

As the October 2015 deadline to adopt ICD-10 codes officially becomes the new standard in just five months, all healthcare providers will be forced to transition their ICD-9 codes over to the ICD-10 codes. For many healthcare providers this transition will prove to be time consuming and complex.

MD Logic to Support ICD10 and ICD9

MD Logic, the leading U.S. provider of High Performance Electronic Health Records Solutions, will release the companies ICD-10 module in May 2015. The ICD-10 module was initially released during the MD Logic EHR User Conference held in June 2014 in Atlanta, Georgia. Prior to the 2014 EHR User Conference the federal government announced the delay of ICD-10 to October 2015. The initial date was scheduled for October 2014.

How to Manage Patient CDs with MD Logic DICOM Technology?

As MD Logic prepares to attend the 2015 AAOS conference in Las Vegas this month, we are reminded of the vast number of clinics that continue to store their patients CDs (containing the outside images of MRIs and CTs) in boxes. Like most clinics in this country, not all MRI or CT images are generated in the physician’s office. Accessing these images online is preferred but is still the minority when compared to most MRI centers providing the patient with the images on CD.

Why Lowering the Bar for EHR Reporting Hurts Doctors?

CMS is implementing a new rule that goes into effect this Spring that will reduce the attesting period from 365 days to 90 days. According to Patrick Conway, M.D., deputy administrator and chief medical officer for CMS, this new rule would update the Medicare and Medicaid EHR incentive programs and whittle the attesting reporting period down to 90 days from the previous 365 days.

Chris Andrews – Rocking Big Daddy 1953-2015

On January 1st one of our most beloved MD Logic representatives and true friend passed away due to an unexpected illness. Born in Texas, Chris relocated to Pensacola Florida in the 1990’s and began selling financial systems to physician and dentists. In 1996, Chris was first introduced to MD Logic. Then in 2000 Chris accepted the position and became an MD Logic sales representative. Over the next fifteen years, Chris participated in MD Logic trade shows throughout the USA.

Meaningful Use Stage II and Smart Phones the Focus of 2014

MD Logic, the leader in high performance EHR solutions, accomplished the goals set forth by the company for 2014. The primary focus of 2014 was to complete the software development and testing of Stage II Meaningful Use certification. Once completed, the next goal would be to train and educate our customers on how to efficiently implement these new government requirements without losing productivity.

35% of Medicare Eligible Professionals Failed to Meet Meaningful Use

Starting January 2015, roughly 257,000 Medicare eligible professionals will be hit with a 1 percent penalty to their Medicare Physician Fee Schedule. The penalty is a direct result of failing to meet Meaningful Use by Oct. 1, 2014. CMS will begin mailing letters this week informing these healthcare providers of the penalty. Instructions regarding the penalty will be posted to CMS website.
