CCD document – what is a CCD document? Funny but most doctors that have been attesting for Stage I Meaningful Use have no idea what a CCD is or have even provided a CCD document for their patients. In order to stay in compliance with Stage I Meaningful Use, a provider must generate a CCD (Continuity of Care) document to each patient that requests a summary for their office visit. CCD documents are the new standard in providing clinical information about a patient.

Congress has put a hold on the proposed 2012 Medicare cuts that would reduce Medicare reimbursement as much as 27.4% to physicians. Congress approved a bill that would delay the proposed cuts from January 1, 2012 to March 1, 2012.
