Are you Providing CCD Documents for Your Patient’s?

CCD document – what is a CCD document? Funny but most doctors that have been attesting for Stage I Meaningful Use have no idea what a CCD is or have even provided a CCD document for their patients. In order to stay in compliance with Stage I Meaningful Use, a provider must generate a CCD (Continuity of Care) document to each patient that requests a summary for their office visit. CCD documents are the new standard in providing clinical information about a patient. Patients can choose to receive a CCD document on paper or request the CCD document be provided electronically. These documents contain the following information:

  • Patients Allergies
  • Patient Diagnosis
  • Patient Problem List
  • Patient Current Medications
  • Patient Past Medications
  • Patient Lab Results

As we move into Stage II for Meaningful Use, CCD documents will become an integral part in the way doctors share clinical information about a patient and their health records. When a patient is referred to another specialist, a CCD document can be sent electronically to the specialist or provided to the patient electronically so that the patient can provide the CCD document to the specialist. Either way, the CCD document can be imported into the patient’s health record automatically providing the patients latest allergies, their current medications, problem list, diagnosis and all lab results to the specialist. This will provide the specialist with the most current clinical information on the patient and eliminate the need for staff members to manually re-enter health information for that patient into the patient’s health records. Upon completion of the visit, a new CCD document can be generated and provided to the primary care doctor to update the patient’s health records with any new lab results, diagnosis, current medications and allergies recorded by the specialist.
